Part 4: After all the required fields have been signed and/or checked, certify the PSSAR DD Form 2962 with a digital or hand signature. If signing by hand, remember to complete the date field.

Once fully completed, send the signed form along with your training certificates to your Hierarchy or Account Manager so that your DISS user profile can be created.
