You can also access a task by searching the subject’s Social Security Number, or SSN, or their Pseudo SSN, or PSSN.

From the DISS Home Screen, type the subject’s SSN or PSSN in the Subject Search field and then click the Search Subjects button.

When the subject’s record appears, click the Subject Details hyperlink beside the subject’s name.

Then click the CSR/RFAs tab to view the Investigation Request CSR task.

Click the Open On button to move directly into that task to review the e-QIP.

The document details contain the Investigation application and release forms.

Click the Investigation application and then click either View or Download. Review the application for completeness and accuracy.

Reviewing Investigation Requests: Search Single SSN/PSSN

Search using Subject Social Security Number (SSN) or Pseudo SSN (PSSN):

From DISS Home, type SSN in the Subject Search field and click Search Subjects.
Click the Subject Details tab.
Click the CSR/RFAs sub-tab to view the Investigation Request CSR task(s).
Click the Open On button to be moved directly into that task and review the e-QIP.
Under Document Details, click the Investigation Application link to View or Download the application.
